A Filipino citizen (natural-born or naturalized), at least 18 years of age, may renounce his/her Philippine citizenship.
Applicants with confirmed appointments and successful online payment must download and print the eReceipt attached in their email. They must present the printed eReceipt to the Consular Officer.
Fill out two (2) copies of the affidavit which can be downloaded from this website. However, DO NOT SIGN the form. It has to be signed in front of a Consular Officer.

Online Payment
Indicate the number of documents to be notarized (Affidavit of Renunciation of Philippine Citizenship and Oath of Renunciation) and pay the notarization fee through the online payment portal.
Applicants who will apply for Renunciation of Philippine citizenship are required to execute an Affidavit of Renunciation of Philippine citizenship and Oath of Renunciation.
Applicants with confirmed appointments and successful online payment will receive an SMS confirmation to the registered mobile number and three separate emails: 1st email to confirm your registration; 2nd email confirming payment; 3rd email with the attached eReceipt which you will need to download, print and bring with you on the date of your appointment.
Proceed to the Consulate on the appointment day and time.
On the appointment day, submit the documentary requirements listed above to the Legal/Notarials Window (Window 1) and sign the Affidavit of Renunciation in front of the Consular Officer.
